EPC G2 Password corruption, then PERMA LOCKED

I`m not sure why but I get allot of EPC-G2 tags that gets perma locked so to speak. The scenario is as follows:
1- I write the tag open EPC number
2- Then I write the EPC PASSWORD and lock the tags..... no problems here.
3- I then issue the EPC password and write a new EPC number into the tag
Still no issues... all works fine HOWEVER
There are cases 10 to 20 tags in 1000 after the EPC number is written can never be unlocked again, it seems the PASSWORD has "corrupted/changed"
I use the IMPINJ Monza 5 chips... I have programmed over 1Million of them and can say this happens allot. I have contributed this to many factors;
but I think the the main culprit is that somehow the PERMA-LOCK bits are being set, once this happens the tag is read only. You can not access that tag again, I also find that too much RF power can overwrite the PERMA LOCK with even having the correct password :( needless to say it`s a pain and I have tried every conceivable way to read the passwords out but no go... it locked
