Dear Rupak,
Nokia and other companies sell mobile phones equipped with RFID readers based on the Near Field Communication (NFC) standard. A number of firms make chips and tags that conform to the NFC standard.
Pricing depends on tag volume and packaging. If you want it in a hardened plastic case, that will cost more than a simple label. I suggest you visit the NFC Forum’s Web site, http://www.nfc-forum.org, for information on companies that manufacture products based on the standard.
