I'm unaware of any system on the market today that employs radio frequency identification to alert patients or doctors to potential heart failure. Several papers have been written describing how such systems might work, however. Here are three:

• Human Cardio-Respiro Abnormality Alert System Using RFID and GPS
Essentially, the idea is that an RFID device could detect abnormalities in the heart, and broadcast that information to a nearby reader. That data could then be communicated to doctors monitoring a patient remotely, over a cell network, thus enabling the physicians to respond quickly and revive the affected individual.
