To my knowledge, hospitals do not offer radio frequency identification implants.

VeriChip was the first company to offer implantation as a service, but that firm was acquired and became known as PositiveID Corp.. It has been reported that PositiveID discontinued implantation services for humans in 2010.

In January 2012, VeriTeQ Acquisition Corp. purchased PositiveID's implantable microchip technologies and the Health Link personal health record system. If you really want to do this to your body—and, frankly, I'm not sure why anyone would—I suggest you contact VeriTeQ.
