What would you recommend for people-tracking applications?
It would not be possible to say which is better, because the first technology might be better in some applications, whereas the latter might be better in others. It would depend on the cost of the tags, the software used to track the tagged patients and so on. One difference between Bluetooth Low Energy (BLE) solutions and 433 MHz active RFID is that 433 MHz systems tend to use time of arrival or other methods to locate a tag to within 10 feet. When BLE is employed for real-time location, a receiver is typically placed in an area, allowing you to know only that a specific tagged object is within a zone in which that receiver is located. So BLE might be less expensive, but it might not give you the location of a specific person.
