Can the information be written to an ISO 15693 tag, with the tag either made read-only or locked via a pass code?

To answer your question, I reached out to James Heurich, the president of RFID Inc., which sells passive LF, HF and UHF RFID systems, as well as active RFID system. James says several ISO 15693 chips have a lock bit, so you could write a serial number to extended memory, change the lock bit and render the memory permanent. The tag has a serial number already, but once you locked the memory, you could use a reader to read the extended memory as the unique serial number in the tag, ignoring the preprogrammed data. This should achieve what you're looking for in an ISO 15693 transponder.
