To my knowledge, electronic component manufacturer Murata Manufacturing Co. makes the world's smallest high-frequency (HF) tag. Murata's LXMS33HCNG-134 tag measures 3.2 millimeters (0.13 inch) in width and length and 0.7 millimeter (0.03 inch) in thickness, which is roughly one-tenth the size of most other HF tags. Murata's version is designed to be both durable and small, with a ceramic substrate enclosing a layered circuit board and an antenna (see Murata Mass-Produces 'World's Smallest HF Tag').
In addition, French RFID systems provider Tagsys demonstrated a 13.56 MHz RFID tag back in 2003 that was about the size of a contact lens (see Tagsys Demos Smallest EPC Tag). I do not know if Tagsys still sells HF tags of that size, but you could try contacting the company to find out.
